Please submit your receipt(s) to MaxL@socalsma.org for reimbursement.

Must be a current and outstanding member!

Information/Attachments needed:

  1. A copy of your current dues receipt

  2. Current mailing address (No P.O. Boxes)

  3. A copy of the purchase receipt

  4. A copy of the actual OSHA card (Certification pdf not acceptable! please wait until you’ve received your actual OSHA card before submitting for reimbursement)
